NDIS Physiotherapy
Welcome to Modern Physiotherapy Clinic! We are a team of experienced physiotherapists who are passionate about helping people with disabilities to achieve their goals and improve their quality of life. We are proud to be registered NDIS providers and offer a range of physiotherapy services under the National Disability Insurance Scheme (NDIS).
Our NDIS physiotherapy services include:

How to Access Services:
Our physiotherapy services are available to NDIS participants who are self-managed, plan-managed, or NDIA-managed. Here's how to access our services based on your NDIS plan management:
- Self-Managed Participants:
If you are self-managed, you can access our physiotherapy services by contacting us directly to schedule an appointment. You will need to pay for our services directly and claim reimbursement through the NDIS portal using your self-managed funds.
- Plan-Managed Participants:
If you are plan-managed, you can access our physiotherapy services by contacting us directly to schedule an appointment. We will work with your plan manager to arrange payment for our services.
The following information will be required for booking an appointment:
- The client’s NDIS number
- NDIS plan start date
- NDIS plan review due date
- Plan Manager’s name (who’d pay for the service)
- Email address for invoice/s
- Line Item/Code (when applicable)
- NDIA-Managed (sometimes referred to as Agency-managed) Participants:
If you are NDIA-managed (Agency-managed), you can access our physiotherapy services by requesting a service booking through the myplace portal or by contacting the NDIA on 1800 800 110. Alternatively you can request a booking by contacting us and providing the following information.
- The client’s NDIS number
- NDIS plan start date
- NDIS plan review due date
- Your contact details
- Name and contact details of the person providing coordination of supports (if applicable)
- A copy of the relevant part of your plan (if you would like assistance)

The length of time it takes to see results will vary depending on your specific condition and goals. Our physiotherapists will work with you to develop a treatment plan that is realistic and achievable.
4. Can I choose which physiotherapist I see?
We will do our best to match you with a physiotherapist who has expertise in your specific condition or needs. However, if you have a preference for a specific physiotherapist, we will try to accommodate your request.
5. What should I wear to my NDIS physiotherapy sessions?
We recommend that you wear comfortable, loose-fitting clothing that allows for ease of movement. It's also a good idea to bring a water bottle and any assistive devices (such as braces or orthotics) that you use regularly.
6. How long will my NDIS physiotherapy session last?
Sessions typically last between 30-60 minutes, depending on your individual needs and goals.
7. Can I bring a family member or caregiver to my NDIS physiotherapy sessions?
Absolutely! We welcome the involvement of family members or caregivers in your treatment process, as they can provide valuable support and encouragement.
8. What is a service agreement?
A service agreement is a document that outlines the terms and conditions of the physiotherapy services we will provide to you as an NDIS participant. It includes information about the goals of your physiotherapy treatment, the services we will provide, the fees and charges for our services, and your rights and responsibilities as a client.
9. What should I expect during my first appointment?
During your first appointment, you will meet with one of our experienced physiotherapists who will conduct a thorough assessment of your needs and goals. This may include a review of your medical history, a physical examination, and an assessment of your mobility, strength, and balance. Based on this assessment, your physiotherapist will develop a personalised treatment plan that is tailored to your specific needs and goals. This appointment is also where you develop your service agreement. Once you’ve completed this initial appointment and have signed your service agreement, you can start on your physiotherapy journey.
10. Can I make changes to my service agreement if my needs or goals change?
Yes, we understand that your needs and goals may change over time, and we are happy to work with you to modify your service agreement as needed to ensure that you are receiving the best possible care.
